Free Group Personal Accidental Insurance (GPAI)

THE MAIN FEATURES OF THIS INSURANCE COVER ARE AS UNDER:

POLICY DURATION: The GPAI policy will be effective from 07-06-2025 to 06-06-2026.

COVERAGES

  • GPAI-DEATH COVER of ₹ 25 lakh (EXISTING)
  • AIR ACCIDENT INSURANCE of ₹ 25 lakh* (ADDITIONAL)
  • PERMANENT TOTAL DISABILITY of ₹ 10 lakh (ADDITIONAL)
  • PERMANENT PARTIAL DISABILITY of ₹ 5 lakh (ADDITIONAL)

*In case death occurs due to Air Accident, the claimant will be eligible for a claim amount of ₹ 50 lac (₹ 25 lac death cover + ₹ 25 lac Air Accidental Cover).

ELIGIBILITY:

  • A permanent employee of the UT Government of J&K and LADAKH including those in Corporations, PSUs, Autonomous bodies, local bodies, universities, and police personnel/SPOs.
  • Aged between 18 to 60 Years or as per the maximum age of superannuation of the employee determined by the service rules in vogue.
  • Must draw their salary through J&K Bank.
  • Employees drawing salary from banks other than J&K Bank will not be eligible under the GPAI policy. However, such employees who open and maintain a salary account with J&K Bank during the policy period shall become eligible for coverage under this GPAI policy.

CLAIM PROCESS

Claimants should approach the respective J&K Bank branch where the deceased employee was maintaining his/her salary account for claim intimation.

Required documents include police report, post-mortem report (wherever applicable), viscera report (if required), death certificate, claim form, LPC, KYC documents, and other relevant documents based on the nature of claim.

Q 1. Which insurance Company has provided the Group Personal Accidental Insurance cover in favor of Govt Employees?

Ans: The Bank has arranged free Group Personal Accidental Insurance cover in favor of Govt Employees from BAJAJ ALLIANZ GENERAL INSURANCE COMPANY (BAGICL) from 07.06.2025 to 06.06.2026 with following coverage’s.

Q.2 who all are covered?

Ans: The GPAI covers all the Permanent employees of the UT governments of J&K and Ladakh including those in Corporations, PSUs, Autonomous bodies, local bodies, universities, and police Personnel/SPOs.

Q 3. Are Terrorism/snake bites/ Animal bites also covered?

Ans: The policy for Group Accidental Insurance covers Death Cover, Air Accident Insurance (Death), Permanent Total/Partial Disability, including claims due to terrorism and snake/animal bites, on a 24/7- Worldwide cover

Q 4. How to lodge a claim?

Ans: The claim form along with all the requisite documents shall be submitted at the branch where the deceased employee has been maintaining his/her salary account or directly to insurance Company. Branch shall forward soft copy of the same to the insurance company via mail and retain the hard copies at Branch.

IN CASE OF ACCIDENTAL DEATH

  • Duly filled PA form (Format Annexed)
  • Affidavit from Legal heirs of deceased (Format Annexed)
  • Indemnity bond (Format Annexed)
  • FIR/FPR
  • Post Mortem (Wherever applicable)/Viscera Report (if required as per PM report)
  • KYC of deceased (address & identity proof)
  • Salary Slip of deceased (LPC)
  • Salary A/C Statement of deceased maintained at JKB Branch with recent salary credit.
  • KYC of claimants (address & identity proof)
  • Joint Account statement of Claimants reflecting title of account, IFSC code etc.
  • Legal heir certificate.
  • Guardianship Certificate/Court order if claimants or any one of the claimants is a minor.
  • Treatment notes (wherever Applicable)
  • Death certificate/death Summary
  • Any other relevant documents depending upon nature of Accident.

IN CASE OF PERMANENT PARTIAL DISABILITY AND PERMANENT TOTAL DISABILITY

  • Duly filled PA form (Format Annexed)
  • KYc of insured/LPC/Salary account statement maintained at J&K Bank.
  • FIR/FPR
  • Duly filled Medical Certificate attached in the Group Personal Accident Claim Form.
  • X-ray films /Investigation reports supporting the diagnosis.
  • Permanent Total Disability or Permanent Partial Disability Certificate from the Government authority certifying the Disability of the insured.
  • Photograph of the patient before and after the accident to Support the disability
  • Any other relevant documents depending upon nature of Claim

Q 6. In case any employee has obtained similar cover on his own, will there be any impact on benefits under this policy?

Ans: Personal Accidental Insurance Policies are benefit policies, thus the sum payable under this policy shall be over and above the paid/payable under other Insurance Policies.
